its really underextruding, my Ender 5 plus when it uses default rotation distance of 33.83 it prints like that, but that printer has a direct drive extruder now so the rotation distance is 7.53 and with that setting prints perfectly.
I think I have obtained the necessary extrusion speed by changing the extruder pressure until 50mm and 100mm give that by putting marks on the filament. I’m now trying the models again to see the difference.

As a retired g I ever I can suggest that the only time this 3 degree tilt of the print head would make no difference would be if the nozzle and CR touch are in alignment along the X axis otherwise the nozzle would be too high. If the KE is anything like SE then there is no real way to correct the Z offset anyway as far as I can see.
