Can someone please confirm or correct me is this a nozzle temperature issue?
Hard to say without more info. Was that printed with the text down on the build plate? What material and temps did you use? Which printer?

You are fast becoming my saving grace lol, it was Ender PLA black 190 DegC and yes face down the original was a battery symbol but I wanted to put the type of batteries on the front (CR123A)
I suspected as much. 190 does seem a bit low for pla - I usually run 200-220 for most brands. I haven’t used the Ender PLA myself. I would also click on 1 wall for first layer so you don’t have all those rings around the box and the text, arachne wall generation as they are better for text, and print thin walls to help with the little serifs. Fill in small gaps on top/bottom might help too. The underextrusion could be just from too low a temp, or it could be flow rate too. Try raising the temp first and adjust from there.
